    By signing below, the above-named resident agrees to the following:
    1. If animal is a dog, it must be properly trained regarding elimination needs and any other potential damages to the rental unit. If a cat, it must be trained to use a litter box. Dogs must be toileted outside. No inside “pads” or artificial turf or tarps on decks or patios are permitted for animal waste.

    2. Resident agrees to be responsible for cleaning up animal/dog waste on premises immediately. Cat litter box must be cleaned daily.

    3.Dogs must be at least one year of age. This must be confirmed by documentation from a veterinarian. If you have a dog younger than 1 year - non refundable $200.

    4. Cats must be spayed/neutered. We do prefer to have cats front paws declawed.

    5. Management must be provided with proof that all required vaccinations have been given. Updates on required vaccinations and other appropriate veterinary care must be provided upon request. A signed verification from the veterinarian must be emailed to management.

    6. Animal must be properly licensed within the city of Winona and proof must be provided to management (Pet licenses are required for all dogs and cats over the age of 6 months in Winona. Cost is $12/year. Call 507-457-8262 or visit www.cityofwinona.com for more information).

    7. Resident will be responsible for any damage caused by animal. Any damages will be itemized and deducted from the security deposit.

    8. If management receives complaints from any other individuals regarding noise, intimidation, health problems, or sanitary issues as a result of the animal or in relation to any of the above items, resident is in violation of this agreement and may be subject to a $500 fine, eviction and/or removal of animal.
